Hospice Chaplain - UCM Home Care - Full-Time, Days

We have expanded our home care services to keep pace with shorter hospital stays, evolving technology, changing insurance reimbursement and a growing elderly population. This position is a 100% onsite opportunity, and you will be traveling in the field between patient's homes. You will need to be based in the greater Chicagoland area. 

The Hospice Chaplain is responsible for the provision of spiritual care services to patients/families/caregivers of Ingalls Hospice, either directly or through coordination of care with other spiritual counselors.

Essential Job Functions

• Assures spiritual assessment of patients/families/caregivers in the hospice program and that appropriate services are rendered in a timely manner. 

• Facilitates the development in the Interdisciplinary Team Plan of Care to meet identified spiritual needs. 

• Provides direct spiritual care to patient/families/caregivers. 

• Serves as liaison and support to community chaplains and spiritual counselors. 

• Maintains records of spiritual care services utilization and related activities for quality assurance, program development, and policy and procedure review and revision. 

• Documents direct services and ongoing communication with community chaplains and spiritual counselors. 

• Attends patient care conference and Interdisciplinary Team meetings as a member of the Interdisciplinary Team. 

• Provides consultation, education, and support to the Interdisciplinary Team on spiritual care. 

• Establish and maintain relationships with community chaplains, spiritual counselors, and spiritual care volunteers to adequately meet patient/family/caregiver needs. Including offering education about hospice care. 

• Assists with funeral or memorial services for patients as appropriate. 

• Plans periodic memorial services to meet the needs of families, personnel, volunteers, and community, including but not limited to Annual memorial and individual family needs 

• Accepts and performs other related duties and responsibilities as required. 

• Commits to one hundred percent (100%) patient and customer satisfaction by always exhibiting a courteous and helpful manner during interactions with others, including patient’s/client’s, families, visitors, physicians, students, and co-workers. 

• Other duties as assigned. 

Continuing Education Requirements: 

UChicago Medicine Home Care personnel are expected to participate in appropriate continuing education as may be requested and/or required by their immediate supervisor. In addition, personnel are expected to accept personal responsibility for other educational activities to enhance job-related skills and abilities and as required by licensing standards.

Required Qualifications 

• Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree). 

• Experience working with death and dying, individuals/family/caregiver. 

• Member in good standing with own religious denomination. 

• Satisfactory completion of at least one accredited CPE program. 

• Ability to work as member of Interdisciplinary Team. 

• Comfort in an interfaith setting. 

• Ability to accept different lifestyles, cultures, beliefs, and values. 

• Ability to network with community clergy and congregations. 

• Knowledge of and commitment to hospice philosophy of care. 

• Hospice experience preferred.
