Valley’s EPIC residential program provides individuals with the support, structure, and resources needed to build a foundation for lasting recovery and independence. As a Residential Case Manager, you'll help clients navigate services, achieve personal goals, and overcome barriers through targeted case management, psychosocial rehabilitation, and ongoing support. Your work will play an essential role in empowering clients to make meaningful progress and move toward a healthier future.

$2,000 Retention Sign-on Bonus!

Pay:  dependent on Case Manager certification status 

• Uncertified starts at $17.50 - Uncertified Case Managers will obtain certification within the first 120 days and pay will be increased
• Certified range starts at $19.61 - Certified Case Manager pay is calculated based on years of certified experience.

Shift Differential: additional $2/hour when clocked in for 3+ hours between 3pm-11pm

Schedule: Tuesday - Saturday | 4:00pm-12:00am (midnight)

Job Summary

Case Managers provide psychosocial rehabilitative services as indicated by the client’s care plan to support clients in meeting their individualized goals. Case Managers are responsible for utilizing evidence-based practices, meeting fiscal responsibilities, and ensuring that regulatory compliance and organizational standards are met.

Essential Functions

• Provides psychosocial rehabilitative services as described by the Medicaid manual
• Supports clients in following individualized care plans and identifying case management goals
• Completes documentation on services provided within Valley’s standards
• Meets productivity expectations as assigned by supervisor
• Distributes medications as necessary
• Transports clients to appointments as necessary

Licenses/Certificates

• Case Manager certification (Post-hire as needed — required within 120 days of start date) 
• CPR certification (Post-hire) 
• Valley de-escalation certification (Post-hire)
