Under the general supervision of the Clinical Supervisor of the Roving Team, the Clinical Social Worker I/II (CSW I/II) works doing in-person outreach in the single room occupancy hotels (SROs) in San Francisco’s Tenderloin, South of Market, and Mission neighborhoods.  The candidate will seek to engage high risk mentally ill, medically complex and/or substance abusing clients to support the enrollment of PSH residents in CAAP, SSI/SSDI, and Medi-Cal so that residents can more easily access benefits that promote their overall wellbeing, and PSH providers can address chronic rent arrears in PSH.

Under this grant, Citywide Clinical Social Worker shall support residents through the benefits enrollment process, including scheduling appointments; accompanying residents to appointments; assisting with the completion of required paperwork; securing documentation, e.g., medical records or income records; referring residents to other sources when needed, e.g., lawyers; communicating with HSA, HSH, and PSH providers about resident status in the benefits enrollment process; providing data about the progress of their efforts; surfacing and documenting opportunities for improving the enrollment processes. Citywide will also participate in monthly meetings with the Lead Consultant, and quarterly, in-person convenings of frontline staff, HAF, and City departments focused on reviewing outcomes, comparing approaches, and identifying what is and is not working across different client populations. Citywide will also conduct trainings for PSH case managers and support the creation of a written manual (for use by PSH case managers) that explains the benefits enrollment procedures.

The individual will hold a caseload of clients for whom they will be providing community-based clinical case management focused on benefit enrollments but also including necessary assessment, treatment planning and crisis intervention.  The CSW will provide coordination and collaboration with property management and support services staff at the housing sites, advocacy for services, communication with families, and mandated reporting as needed.  They will coordinate and consult closely with acute hospitals settings and outpatient medical providers to increase the continuity of care for their clients.  They will track and document their interventions, assessments and outcomes in a timely manner.  The candidate will also perform other duties as assigned.

The Supportive Housing Teams have a two-hour weekly clinical team meeting where we foster a supportive learning and working environment by engaging in clinical trainings, sharing community resources, peer support, and presenting clients to the team for feedback and support. There is also an hour of clinical supervision weekly to each clinician.  The CSW I/II should possess a basic understanding of the DSM V mental health diagnostic criteria, Harm Reduction principles, Trauma Informed Care and Anti-Racism. You will be working with diverse and multicultural teams, clients, and community, the incumbent will participate and contribute to culturally focused clinical interventions and will practice a commitment to cultural humility and anti-racism.

The job is based in the community and requires frequent outreach.  The CSW will need to possess the ability to lift 50 lbs, walk several miles a day and climb stairs when elevators are not available.
