Job Title: Social Worker - Adult Metabolic Service (PART-TIME 0.6 FTE)

Social Work Services at the Mount Sinai Hospital

The Mount Sinai Hospital is a 1,134-bed facility with an extensive outpatient and specialty care network and is the largest hospital in the Mount Sinai Health Care System. It offers comprehensive social work services and programming to meet the myriad needs of the diverse populations we serve. We believe that total patient care must emphasize the physical, emotional, and social needs of each patient and their care partners. Social Workers collaborate within interprofessional teams to serve patients and the larger community from both a micro and macro level including both direct care and prevention. Founded in 1907, the Department of Social Work Services at the Mount Sinai Hospital is one of the oldest hospital social work departments in the nation with over 450 licensed social workers employed across more than 65 different program/service areas. Employment in our department provides a pathway to LCSW licensure as well as access to a broad range of continuing education, professional development and extra-curricular opportunities.

The social worker in this .6 FTE position (Monday, Tuesday, and Thursday workdays) will provide social work services to adult patients that are diagnosed with an inborn error of metabolism as children.  Inborn errors of Metabolism require life-long management and treatment with specialized diets and medications; as therapy and treatment have advanced, many patients are living longer lives into adulthood.  Some disorders are prone to catastrophic crises while others are more chronic in nature.  The social worker will provide short and long-term individual and family care through psychotherapy, care management, crisis intervention, and health education as an active member on the interdisciplinary medical team.  The social worker will follow these patients primarily in the outpatient clinic setting. There can be end-of-life and bereavement care associated with both aspects of this role, and the social worker should be prepared to support the patient/family through the process.  Strong advocacy in keeping with a patient’s evolving medical and developmental needs strongly underpins all aspects of this work.

This position is based in-person in the outpatient Metabolic clinic, however at times this worker will also liaise with adult inpatient Social Workers for continuity when patients are hospitalized. This includes, but is not limited to, helping with unique metabolic specific resource needs, medications, and formulas. There will be limited cross-coverage with the Pediatric Metabolic Social Worker during PTO.
