At Valley CTOS, Case Managers work alongside clients to identify what matters most to them and build the skills, resources, and confidence to get there. Through individualized support and psychosocial rehabilitation, you’ll help clients overcome barriers, strengthen independence, and turn personal goals into meaningful progress.

Pay: Dependent on Case Manager certification status

• Uncertified starts at $17.50 — Certification will be obtained within the first 120 days and pay will be increased to the certified rate.
• Certified range starts at $18.75 — pay is calculated based on years of certified experience. 

Schedule: Monday – Friday | 8:00am – 5:00pm

Job Summary

Case Managers provide psychosocial rehabilitative services as indicated by the client’s care plan to support clients in meeting their individualized goals. Case Managers are responsible for utilizing evidence-based practices, meeting fiscal responsibilities, and ensuring that regulatory compliance and organizational standards are met.

Essential Functions

• Provides psychosocial rehabilitative services as described by the Medicaid manual
• Supports clients in following individualized care plans and identifying case management goals
• Completes documentation on services provided within Valley’s standards
• Meets productivity expectations as assigned by supervisor
• Distributes medications as necessary
• Transports clients to appointments as necessary

Licenses/Certificates

• Case Manager certification (Post-hire as needed — required within 120 days of start date) 
• CPR certification (Post-hire)
• Valley de-escalation certification (Post-hire)
