Position Summary: Serves as Liaison between physician and patient.  Provides patient clinical education, reviews lab results and discusses with patient, calls in prescriptions to pharmacies.  Also provides coordination of care following hospital discharge.  Provides skilled nursing care, tasks, and procedures under the direction and supervision of a physician, or other authorized healthcare provider, including administration of medication, and recording of vital signs.
