DaVita is looking for a PER DIEM Licensed Social Worker (LISW) (LICSW) (LGSW) to support our patients and their families at our Cass Lake Dialysis Clinic in Cass Lake, MN!

As a Social Worker (LISW) (LICSW) (LGSW) at DaVita, you’ll be a part of a Team that values work-life balance and where your personal and professional growth is a top priority.

Here is what you can expect when you join our Village as a Clinical Social Worker (MSW):

• Covering Cass Lake Dialysis clinic only
• Will need to be in clinic at least twice a month
• Per Diem status - fewer than 24 hours a week
• Training - 6-8 weeks
• 50 patient case load
• Will provide group supervision 4 hours a month that is no cost to the teammate

• Life on dialysis can be a difficult transition for many patients-and you'll be there to support and advocate for them.
• Help patients understand their rights and responsibilities, and guide them in managing the physical, mental, emotional and financial demands of End Stage Renal Disease.
• Build meaningful and long-term relationships with patients and their families in an intimate outpatient setting.

Requirements:

• Licensed or registered in the state of practice as required by state regulations -- (LISW) (LICSW) (LGSW)
• Meets all state required regulations to practice in the dialysis setting.
• Master's degree in Social Work (MSW) required with a specialization in clinical practice
• Demonstrated knowledge of government and private insurance programs
• Basic computer skills in MS Word, Excel, PowerPoint and Outlook as well as functional proficiency with DaVita specific applications within 60 days
