Nexus Children’s Hospital – Shenandoah is seeking a PRN Pharmacist to support medication management for pediatric and adolescent patients with complex medical, rehabilitation, and behavioral health needs. This opportunity is ideal for a hospital pharmacist who values interdisciplinary collaboration and understands the precision required when caring for children.

Pediatric pharmacy experience is strongly preferred, particularly experience reviewing weight-based dosing, evaluating age-appropriate therapies, and supporting patients with complex medication regimens.

Position Summary

Under the supervision of the Director of Pharmacy, the Pharmacist supports daily pharmacy operations and provides clinical pharmacy services that promote safe, effective, and appropriate medication use. This position collaborates with physicians, nursing staff, and other healthcare professionals while ensuring compliance with hospital policies, accepted standards of practice, and applicable DNV, CMS, state, and federal requirements.

What You’ll Do

Pediatric Medication Safety & Clinical Care

• Review and interpret physician orders for accuracy, appropriateness, dosing, drug interactions, contraindications, and therapeutic duplication.

• Evaluate pediatric medication regimens with attention to weight-based dosing, age, organ function, formulation, and route of administration.

• Review patient charts to ensure prescribed medications are appropriate for each patient’s diagnosis, condition, and treatment goals.

• Process medication orders; prepare and dispense medications; and monitor medication use and administration.

• Monitor drug therapy and medication levels, adjusting therapy within authorized protocols or communicating recommended interventions to the treating physician.

• Support formulary use, approved therapeutic substitutions, and IV-to-oral or feeding-tube conversions when clinically appropriate.

• Collaborate with physicians to identify clinically appropriate, cost-effective medication alternatives.

• Educate patients and families about medication therapy and document counseling, interventions, and clinical activities.

• Participate in interdisciplinary team conferences to support coordinated care.

Pharmacy Operations & Compliance

• Maintain a clean, organized pharmacy and support safe medication preparation, storage, distribution, and inventory practices.

• Support Pyxis operations, including nursing staff access, medication database maintenance, and resolution of unknown medication descriptions.

• Review and reconcile daily reports, including medication discrepancies and overrides.

• Ensure medication charges and related documentation are accurate and daily charge reports are generated.

• Follow hospital and department policies and applicable accreditation and regulatory requirements.

• Communicate operational concerns and resource needs to the Director of Pharmacy.

• Participate in performance improvement activities that strengthen medication safety and pharmacy efficiency.

Collaboration & Staff Education

• Serve as a medication information resource for physicians, nurses, patients, families, and other hospital staff.

• Support orientation, training, competency assessments, and ongoing medication-related education for pharmacy and nursing personnel.

• Provide educational in-services to clinical staff.

• Communicate professionally with colleagues, vendors, and patients’ families.

• Participate in department meetings and perform other duties as assigned.

Qualifications

Education

• Graduate of an accredited College of Pharmacy.

• Advanced degree in a related field and/or completion of an accredited hospital pharmacy residency preferred.

Experience

• Pediatric hospital pharmacy experience strongly preferred.

• Three years of hospital pharmacy experience preferred.

• One year of pharmacy management or supervisory experience preferred.

• Knowledge of DNV, CMS, and applicable state regulatory requirements.

• Strong clinical judgment, attention to detail, communication skills, and ability to collaborate with an interdisciplinary team.

Licensure & Certifications

• Current Texas Pharmacist license.

• IV certification.

• Current CPR certification.

PRN Scheduling

This position provides coverage based on hospital and department needs. Availability for after-hours or on-call coverage may be required.
