Health Navigator - Job Overview

Schedule: Monday-Friday (8AM-5PM)

As an In-Clinic Health Navigator, you will build meaningful relationships with community members and Legacy patients while helping them access medical care, stay connected to treatment, and adhere to medications for HIV prevention or treatment through Legacy Pharmacy partnerships. Your compassion, communication, and cultural awareness will help patients move confidently through the continuum of care and make informed decisions about their health.

You will serve in both clinic and community settings, connecting individuals who need prevention services with Legacy care. As an integrated member of the clinical care team, you will bring health education and practical navigation support to conversations about HIV/STI testing, Pre-Exposure Prophylaxis (PrEP), Post-Exposure Prophylaxis (nPEP), HIV Treatment as Prevention (TasP), medication adherence, and safer sex practices.

This is an opportunity to join a collaborative healthcare family where your passion for patient support can make a lasting difference in community health. You will work alongside an interdisciplinary team and community partners while helping patients and families build confidence and independence in managing their health. Position may be partially or fully funded by grant(s).
