Make a Difference Every Day

Are you passionate about helping older adults maintain their independence and quality of life? At HopeWest PACE, you'll build meaningful relationships with participants while working alongside an interdisciplinary team dedicated to providing exceptional, whole-person care.

Whether you're an experienced Occupational Therapist or a new graduate ready to begin your career, we'd love to meet you.

The Occupational Therapist (OT) evaluates, plans, and implements individualized treatment programs to help patients develop, recover, or maintain daily living and work skills. This role supports participants in achieving optimal independence, safety, and quality of life through therapeutic interventions aligned with interdisciplinary care plans. The OT provides a combination of home-based safety assessments, skilled therapeutic interventions in the day center, and coordination of durable medical equipment (DME) and other supportive services to promote participant independence and quality of life.

Why Join HopeWest PACE?

As an Occupational Therapist, you'll evaluate, develop, and implement individualized treatment plans that empower participants to safely remain in their homes and communities. You'll help improve strength, mobility, daily living skills, and confidence while collaborating with physicians, nurses, therapists, social workers, and other healthcare professionals to provide comprehensive care.

Every day brings the opportunity to make a lasting impact on someone's independence and well-being.

What is PACE?

PACE (Program of All-Inclusive Care for the Elderly) is a nationally recognized Medicare and Medicaid program that provides comprehensive medical and supportive services for older adults who wish to continue living safely at home.

As the population of older adults continues to grow in the Grand Junction area, the need for compassionate healthcare professionals has never been greater. Joining HopeWest means becoming part of a mission-driven organization that is transforming the way seniors receive care.
