The Licensed Practical Nurse works as an integral member of the team providing services at agency locations across a range of service lines. The primary goal of health monitoring is to identify clients at risk due to poor physical health and medical conditions, improve their health outcomes through early identification and care, and prevent lowering their quality of life or early death.

Health monitoring goals include: providing health assessments and health monitoring to a wide range of agency clients, proving monitoring services to high risk clients weekly, with the aim of reducing medical hospitalizations.
