PROGRAM SUMMARY

Beginning July of 2026, all Behavioral Health Plans in California will be required to implement High-Fidelity Wraparound (HFW) to fidelity under the Full Service Partnership (FSP) program. All FSPs for children and youth must align with the principles, values, and standards of the HFW model.

HFW provides a comprehensive, holistic, evidence-based, child/youth and family-driven process for responding when children and youth experience significant mental health or behavioral challenges, including children and youth involved in multiple child/youth serving systems. HFW is a team-based and family-centered evidence-based practice that includes an "anything necessary" approach to care for children and youth with the most intensive mental health or behavioral challenges

POSITION SUMMARY

The Facilitator ensures that the Wraparound process is implemented with fidelity and integrity, consistently maintaining hope that each family can achieve a better future. This role is responsible for initiating engagement with parents and youth, forming the Child and Family Team (CFT), developing safety plans, and creating an individualized CFT plan based on family strengths, needs, and preferences.

Throughout the process, the Facilitator maintains a non-judgmental, strengths-based, and culturally responsive approach. They coordinate the development and ongoing revision of CFT plans, reflecting lessons learned from what has and has not worked. From the beginning of care, they also plan for transitions, ensuring smooth movement toward independence and stability. 

The Facilitator provides leadership to the team, consistently demonstrating Wraparound values and principles. They engage others in a shared mission, display strong interpersonal skills, and effectively navigate the complex systems of public services. They also build strong connections among CFT members, valuing diverse perspectives that support family-driven success. Additionally, the Facilitator provides direct services and interventions aligned with the Wraparound plan. This position may be expected to participate in an on-call rotation.

GENERAL REQUIREMENTS:

Education/Experience

• Bachelor’s Degree in social work, psychology, nursing, occupational therapy, or a related field from an accredited college or university preferred.
• One year experience in human services, preferably case management.
• Must qualify as a Medi-Cal Specialty Mental Health Services (SMHS) provider. Eligible activities may be claimed under Medi-Cal if all state and federal Medicaid requirements are met.
• Required Training - Completion of the Statewide Standardized Child and Family Team (CFT) Facilitation Training, accessible through Regional Training Academies, the UC Davis Resource Center for Family-Focused Practice (RCFFP), or CACWT

Licensure, Registration, Certification

• Valid CA driver’s license and proof of automobile insurance.
• Completion of CPR/First Aid/ProACT certification within the first thirty (30) days of employment.

Preferred training/experience in the following Evidenced Based Practices: 

• Intensive Case Management (ICM)
• High Fidelity Wraparound (HFW)

Requirements

JOB DUTIES & RESPONSIBILITIES:

Actively participate in the engagement process by providing reassurance that the family’s circumstances and support the team in identifying and celebrating achievements, growth, and strengths throughout the Wraparound process.

During Team Meetings (Child and Family Team Meetings/Wraparound Team Meetings)

• Schedule, convene, and facilitate Wraparound team meetings, ensuring to invite natural and formal supports, guide the family’s team through meaningful participation
• Guide the Wraparound team through needs discovery, strengths identi?cation, and family vision articulation
• Lead development of Wraparound team agreements, team mission, the family vision statement, and development of the family’s goals
• Prioritize needs and translate them into measurable outcomes in the Plan of Care
• Coordinate the development of crisis and safety plans that re?ect family-driven, proactive, and reactive strategies
• Update and review plans ensuring all team members are aligned and aware of responsibilities

Outside of Team Meetings

• Maintain ongoing communication and follow-up with all Wraparound and Child and Family Team members to ensure clear communication, accountability, and service delivery
• Communicate with the Child and Family Team (CFT) including accessing the family’s IP- CANS ratings to incorporate into planning and to provide ongoing updates as well as receive emergent information from the CFT
• Engage additional natural supports and professionals to expand the Child and Family team’s capacity
• Support transition planning from the onset, guiding the family through benchmarks toward sustainable support and success
• Incorporate tools such as the IP-CANS to inform planning and monitor progress

Participate as needed in the implementation of safety plans in collaboration with clients, their support systems, and the treatment team. 

Productivity & Service Billing 

• Meeting expectations in Productivity and Service Billing skills focus on the ability to hit program productivity expectations on a monthly basis.

This is further demonstrated by adhering to scheduled shifts, appointments, meetings, and other work-related obligations, minimizing unscheduled absences, and adjusting work schedule to accommodate program need.

• Meeting expectations in the Collaboration and Teamwork competency includes working collaboratively with multidisciplinary teams, including psychiatrists, case managers, educators, outside agency providers and other mental health professionals, to coordinate care and provide integrated services. This skill is also demonstrated by communicating effectively with clients' families and caregivers, involving them in treatment planning and facilitating their understanding of mental health issues and interventions.

Interpersonal Communication Skills

• Meeting expectations in Interpersonal Communication Skills focuses on establishing rapport quickly, understanding and addressing client’s needs, soliciting and using input from team members and outside agencies, communicating data and information, and facilitating communications among outside agencies are other stakeholders. Communications are clear, concise, respectful, effective, and timely.

Quality of Care

• Quality of Care encompasses the ability to consistently deliver work and client care of exceptional quality while ensuring precision, thoroughness, and adherence to established standards and procedures. This competency is essential in maintaining the integrity and effectiveness of organizational processes and services and ensuring clients are receiving the best level of care possible.

Accuracy and Quality Improvement 

• Meeting expectations in Accuracy and Quality Improvement includes upholding status of a single signer (40% or lower return rate for three consecutive months) by completing accurate documentation with minimal errors. Learning and correctly using Bells AI and its features for progress note completion. Actively participate in departmental and agency-wide quality improvement activities. (For programs who have not yet rolled out Single Signer, the metric will be a return rate of 40% or lower). 

Efficiency, Time Management, and Organization

• Meeting Expectations in Efficiency, Time Management, and Organization skills encompass the ability to take responsibility for one’s performance by spending time appropriately distributed to ensure that both internal and external client needs are met. Organizes and prioritizes assignments to ensure that newly emerging, urgent issues are resolved, while not losing sight of longer-term goals. Documentation efficiency skills encompass the ability to submit accurate and complete progress notes or other documentation within 3 days. Crisis notes to be submitted within 24 hours. Collaborative documentation is used with clients regularly.

• Meeting expectations in Cultural Competency encompasses awareness of personal cultural biases and assumptions, as well as the cultural backgrounds of clients. Staff respect and value the diversity of their clients, including their beliefs, values, traditions, and customs. PHYSICAL REQUIREMENTS:

• Ability to drive a motor vehicle without restrictions, often in heavy traffic and frequently long distances (one hour or more) to ensure safe transport to client settings and to safely transport clients to appointments as needed.
• Senses of vision, hearing and smell sufficient to regularly operate a motor vehicle, to visualize threats in the environment or risks to clients, and to operate safely in the field.
• Required to occasionally lift and carry 10-20 pounds.
• Regularly required to sit, stand, climb stairs (1 or more flights) and walk while in route to and during family visits.
