Registered Nurse (RN) – Hospice House/Inpatient Unit

• Full-time
• 6:30 PM – 7:00 AM
• Location: Inpatient Unit at OSU Medical Center
• Every third weekend required
• Holiday rotation required

About the Role

Join our Hospice House/Inpatient Unit team as a Registered Nurse and provide compassionate, patient-centered care to individuals with life-limiting illnesses. You’ll support patients and their families throughout their hospice journey while working closely with an interdisciplinary team in a collaborative environment focused on comfort, dignity, and clinical excellence.

Key Responsibilities

• Provide skilled nursing care using the nursing process, including comprehensive assessment, planning, implementation, and evaluation.

• Assess and address patients’ physical, emotional, psychosocial, and spiritual needs while promoting comfort, dignity, and quality of life.

• Develop, implement, and regularly update individualized plans of care in collaboration with patients, families, and interdisciplinary care teams.

• Monitor patient conditions, recognize changes in status, and promptly communicate significant findings to physicians and other healthcare providers.

• Evaluate and modify nursing interventions, medications, and care plans based on changes in patient condition and clinical needs.

• Participate in patient admissions, interdisciplinary team meetings, care conferences, and ongoing care coordination.

• Maintain accurate, timely, and thorough documentation of assessments, interventions, physician orders, medications, and patient status in the electronic medical record (EMR).

• Provide clear and effective patient handoffs using SBAR and other standardized communication techniques to ensure continuity and safety of care.

• Provide guidance, supervision, and support to personal care specialists, volunteers, and other caregivers to ensure care is delivered according to established plans and standards.

• Collaborate effectively with physicians, nurses, support staff, and other departments to promote high-quality, patient-centered care.

• Participate in quality improvement initiatives focused on patient safety, outcomes, regulatory compliance, and organizational excellence.

• Serve as a preceptor and mentor for new team members, providing clinical guidance and support during orientation and training.

Qualifications

• Current Registered Nurse (RN) license in good standing with no disciplinary actions.
• Current CPR certification.
• Certified Hospice and Palliative Nurse (CHPN) certification preferred; support is available for eligible employees pursuing certification.
• Strong clinical assessment, symptom management, and care planning skills.
• Experience documenting patient care in an electronic medical record system.
• Valid Ohio driver’s license, reliable transportation, and proof of auto insurance.
• Ability to successfully complete a criminal background check and 10-panel drug screening.
