About the Role

Support the clinical operations that allow hospice teams to deliver exceptional care to patients and families. As a Hospice Clinical Coordinator, you'll serve as the organizational hub for the clinical team, coordinating schedules, maintaining accurate documentation, and facilitating communication that keeps daily operations running efficiently. Your work behind the scenes will help ensure clinicians have the information, resources, and support they need to provide timely, compassionate care.

This role is ideal for a highly organized healthcare professional who thrives in a fast-paced environment and enjoys balancing multiple priorities. You'll coordinate patient scheduling, manage clinical documentation within the EMR, prepare reports and meeting materials, support field staff with operational needs, and collaborate closely with clinical leadership to optimize workflows and maintain compliance. Through your attention to detail, problem-solving skills, and commitment to service excellence, you'll help create a seamless care experience for patients while supporting the success of the entire hospice team.
